Each quarter we will be issuing a new BSafe Incident Case Study on this page. These will be based on a real accident or claim, appropriately anonymised , with a summary of the key lessons learned. BSAFE INCIDENT CASE STUDY 27: GROUNDING OF FERRY. A roll-on/roll-off (ro-ro) ferry lost control while attempting to depart the Port of Calais in severe weather conditions. BSAFE INCIDENT CASE STUDY 28: ALLISION WITH PIER AND DRY DOCK BY CAR CARRIER. The mooring ropes of a car carrier moored at the Port of Bremerhaven parted. This incident caused impact damage to the ship, the berth, and a nearby floating dock. BSAFE INCIDENT CASE STUDY 29: BENZENE VAPOUR FATALITY FOLLOWING UNAUTHORISED TANK ENTRY. While a ship was underway after discharging benzene, a crew member entered a cargo tank before enclosed-space entry procedures had been authorised and before confirming the atmosphere was safe. He was later found collapsed at the bottom of the tank and, despite rescue efforts, did not survive.
